Patient Care Coordinator / Social Worker
Overview
We are searching for a patient care coordinator with a background in social work who can help identify potential patients and guide them through our program – from application and initial screenings to hearing solutions and follow-up care. Our ideal new team member:

Enjoys working with all ages – especially the elderly,

Has excellent management, communication, and people skills, and

Is a self-starter who has experience in community outreach.
Responsibilities
-
Manage all aspects of patient care including application process, appointment scheduling, tracking, and follow-up
-
Responsible for regular reporting to inside and outside partners
-
Assist in community outreach efforts and liaison with partner organizations
-
Communicate as needed with PHC and Pacific Hearing Service staff, caregivers, and outside professionals involved with the patient to assure best possible outcome

Problem-solve as needed to aid in transportation, translation, and any other roadblocks to patient care; manage delivery and repairs

Organize community screening and other educational on-site events (post COVID-19)

Maintain patient information and care history through practice management system software

Organized: Ability to multi-task, be flexible, and track several work streams simultaneously while remaining responsive to client needs

Skilled Listener: Work in partnership with patients, audiologists, and staff to empower and support patient’s hearing success

Engaged: Ability to establish effective relationships with all patients regardless of their challenges and
complexity
Qualifications
-
3 years social work or community outreach experience
-
Interested in working with children, adults, and the elderly
-
Able to attend weekend events occasionally (post COVID-19)

Excellent project management skills – detail oriented
-
Spanish or Mandarin/Cantonese language skills a plus
-
Willing to learn our office management system software
Schedule and Compensation
-
Part-time to start; $28 to $32 per hour
-
Flexible schedule required, with ability to work some evening and weekend hours as needed

Ability to work in our Los Altos office
